Studies have shown that large claims account for over 80% of medical expenditures.

During these times of escalating medical expenses, we need to find ways to save claim dollars. Large Case Management has proven to reduce paid claims substantially, when instituted early, in potentially catastrophic illnesses.

For effective case management, Managed Care Concepts, Inc. accommodates both physicians and patients by actively negotiating prices with their preferred vendors when appropriate or possible. Onsite visits may also be made to evaluate the level of care, quality of equipment and services. The Case Manager will submit monthly reports for active large case management. The reports will reflect true cost savings obtained through actively negotiating discounts on medications, medical equipment and supplies, hospital bills, outpatient charges, home health services, and sub-acute facility charges. Through communication with physicians, healthcare providers and patients, the Case Manager may also realize cost savings by encouraging and facilitating less costly alternative plans of care: for example, home care vs. hospitalization. The fees for large case management are not really expenses but are paid for by the savings to the plan.

In addition to the above diagnosis there are situations where Large Case Management can effect cost savings while enhancing appropriate care. They are:

Case Management intervention may include:

  • Arranging a transfer from the hospital to an extended care facility, nursing home, specialty facility, or home with home health care.

  • Arrangements are made for durable medical equipment, medications and professional services as indicated.

  • Family training and education, as well as counseling are an integral part of the program.

On the average, for every dollar spent for large case management the plan will save twelve dollars.

  • Repeated in-patient admissions, excessive length of stay, or frequent treatment.

The following diagnoses are used as a guideline for determining Large Case Management referrals:

Obstetrics

  • Premature Birth
  • Hospitalization before 32 weeks gestation
  • Premature labor requiring monitoring
  • Pregnancy induced hypertension

Orthopedic

  • Amputations of major extremities
  • Severe crushing injuries of major extremities
  • Multiple fractures/trauma
  • Osteomyelitis
  • Pelvic fracture

Neurologic

  • Amyotrophic Lateral Sclerosis (ALS)
  • Loss of consciousness greater than 24 hours
  • Open and closed head injury
  • Encephalitis/Brain Abscess/Tumor
  • Spinal cord injury/Quadriplegia or Paraplegia
  • Stroke (CVA) with paralysis
  • Muscular Dystrophy or Multiple Sclerosis

Metastic Cancer

  • Any brain, liver, lung, breast, prostrate or stomach cancers
  • Any cancers in the terminal stage
  • Hodgkin’s Disease
  • Kaposi’s Sarcoma
  • Leukemia/lymphoma/multiple myeloma
  • Pediatric cancers
  • Radiation therapy and/or chemotherapy

AIDS/AIDS Related

  • Acquired Immune Deficiency Syndrome (AIDS)
  • AIDS-related complex (ARC)

Cardiovascular

  • Aneurism/Abdominal or Thoracic
  • Cardiomegaly

Psychiatric

  • Inpatient admission
  • Intensive outpatient treatment program

Endocrine

  • Chronic pancreatitis
  • Diabetes

Pulmonary

  • Debilitating emphysema/COPD
  • Pneumocystic carinii pneumonia (PCP)
  • Ventilator patients
  • Cystic Fibrosis

Neonatal/Pediatric

  • All admissions to the neonate ICU
  • Birth weight under 4 pounds
  • Respiratory Distress Syndrome
  • Necrotizing enterocolitis
  • Failure to thrive
  • Growth hormone therapy
  • Scoliosis/Leggs Perth

Renal

  • Renal failure requiring dialysis
